As I say, once you're safely past the prompt filter, your good. The rest of the prompt will impact success, if it's already risky then you may have more fails.

I think that is a bit of a myth. What does happen and is happening is they tighten the filters based on prompt analyis Vs what gets blocked. That said, MS and OAI are continually updating things. There have been a couple of updates this week which broke prompts and also adjusted the number of gens you can make per day again.

I think you're giving them too much credit. I don't doubt that they are actively analyzing outputs and prompts used but I think it's more on a collective data perspective. What has been tested and observed is that there are different configurations of servers with different filtering and limits. This has been shown by logging out and back in on the same account and ending up on a different server. some prompts work, then stop and others start working. You don't even need to log into a different account to observe this.
What is concerning and will no doubt have a massive impact on Bing/Designer very soon are the idiots generating deep fake porn of celebrities and posting all over twitter. This stuff is escalating quickly and won't be long before MS will take drastic steps against the model. I'll be honest, I'm surprised it's taken this long as it is comically easy to work around filters. I wouldn't be surprised to see the models go pay only with verification as a way to mitigate it within the month.
Bear in mind we are actually talking about AI here. I would imagine the companies operating image generators already have AI bots watching every single image that is created and analysing what it contains. And of course those bots are constantly being trained to improve their "problem recognition" skills. But at the same time there's also a commercial advantage in allowing a certain amount of kink content through, because it assists engagement and provides even more training data.
The huge advantage of using an AI bot instead of humans to oversee stuff like this is that the bots can look at everything, every single frame created, as opposed to a human checking one out of ever 20,000 images if they're lucky. And yes, there'll also be AI bots trawling the net for discussions on how to circumvent filters.

This is also able to be seen just in the general layout of the page. Im using mobile, so I have no idea if the desktop version does the same thing or not. But on the mobile version of Bing, sometimes it displays my collections in a drop-down list, and starting here recently, it will sometimes show them as thumbnailed galleries. I have no idea which I will see until I'm signed in.
