I don't believe you are 23. From your user name you are clearly over 100 years old but that doesn't matter because you clearly know how to party. That said, you better celebrate appropriately with 100s of cakes and ice cream cakes.
Wearing your birthday suit is a given... You need to have 23 things happen to you. A combination of 23 will work. -- pies, puddings, syrups, head dunks in a bucket of goo (or water) that totals 23. Of course, you will need a decorated cake with an overload of whipped cream icing that will go straight to your face as the 23rd thing to happen. And get 2-3 friends to celebrate by delivering all these "presents" to you. Happy birthday!
